I am totally confused. Can you explain if the following sentences are correct or not?

- Equipment has requested by Dr. Ahmad to be transferred.

- He was requested for equipment to be transferred.

No. You cannot say that. To change your sentences from an active voice into a passive one, you need to know the TENSE of sentences.

For the following sentence, the tense is in the present perfect:

-        Dr Ahmad has requested for equipment to be transferred. (active in the present perfect)

-       Equipment has BEEN requested by Dr. Ahmad to be transferred. (passive in the present perfect)

 

For this sentence, the tense is in the past:

Dr Ahmad (NO HAS)  requested for equipment to be transferred.

You can say:

- Equipment was requested by Dr. Ahmad to be transferred.

 

So, it depends on the time of sentences you are using.

 

And you cannot say:

He was requested for equipment to be transferred.

because the sentence is in the active voice. You should say:

He requested for equipment to be transferred.

 

So, you need to tell the difference between active and passive voices and the tenses of sentences so that not to be confused.
